The left wing-back, who was arguably in the best form of his career at the time, suffered an ACL injury on November 23 against Juventus in the Champions League.
Following an initial evaluation, the Chelsea medical team decided to wait six weeks to see if the issue could be remedied without the need for surgery.
However, football.london reports that the England international has been informed that he faces a lengthy period on the sidelines.
According to the report, a second assessment was conducted on Monday, with the conclusion that the 25-year-old would require surgery.
This development effectively eliminates Chilwell from contention for the remainder of the season, forcing Chelsea head coach Thomas Tuchel to seek a backup during the winter transfer window.
