Posse\u0301der une copie de ce ge\u0300ne dans son ADN augmente le risque par trois de de\u0301velopper la maladie au cours de la vie. Deux copies augmentent le risque de&#8230; 8 a\u0300 12 fois. Le me\u0301canisme d\u2019action de ce ge\u0300ne, qui jusqu\u2019a\u0300 pre\u0301sent e\u0301tait mal compris, vient d\u2019e\u0302tre en partie mis a\u0300 jour dans une e\u0301tude du MIT publie\u0301e dans Nature. Au c\u0153ur du processus : une mauvaise distribution du choleste\u0301rol dans le cerveau, responsable de la baisse des fonctions cognitives.<\/p>\n<p>En observant le cortex pre\u0301frontal \u2013 le centre cognitif du cerveau \u2013 de 32 personnes de\u0301ce\u0301de\u0301es, les chercheurs du MIT ont observe\u0301 une diffe\u0301rence entre ceux porteurs du ge\u0300ne APOE4 et les autres. Plusieurs lipides semblaient e\u0302tre mal me\u0301tabolise\u0301s. Le choleste\u0301rol en particulier semblait mal traite\u0301 par les oligodendrocytes, des cellules de soutien du syste\u0300me nerveux central. Leur ro\u0302le est de cre\u0301er la mye\u0301line, la gaine compose\u0301e de lipides qui entoure les neurones et permet le passage du courant e\u0301lectrique.<\/p>\n<p>Le signal e\u0301lectrique rompu<\/p>\n<p>\u201cLes oligodendrocytes utilisent le choleste\u0301rol pour synthe\u0301tiser la mye\u0301line et cre\u0301er les diffe\u0301rentes couches qui isolent les neurones. C\u2019est d\u2019ailleurs la\u0300 que se trouve la quasi-entie\u0300rete\u0301 du choleste\u0301rol dans notre cerveau en temps normal. Mais chez les porteurs du ge\u0300ne APOE4, nous nous sommes aperc\u0327us que le choleste\u0301rol s\u2019accumulait dans le re\u0301ticulum endoplasmique (un re\u0301seau de tubules de membranes situe\u0301 dans une cellule, ndlr)\u201d, explique la Dr Li-Huei Tsai, neuroscientifique au Massachusetts Institute of Technology (MIT) a\u0300 Sciences et Avenir.<\/p>\n<p>\u201cLorsque les oligodendrocytes ne parviennent plus a\u0300 transporter le choleste\u0301rol, cela diminue leur capacite\u0301 a\u0300 synthe\u0301tiser la mye\u0301line.\u201d Sans leur gaine de mye\u0301line, le signal e\u0301lectrique ne circule plus dans les neurones. \u201cC\u2019est la mye\u0301line qui permet aux signaux e\u0301lectriques de se de\u0301placer vite, d\u2019arriver au bon endroit dans le cerveau, malgre\u0301 parfois de longues distances a\u0300 parcourir. Lorsqu\u2019il y a moins de mye\u0301line ou qu\u2019elle est abi\u0302me\u0301e, le signal e\u0301lectrique est rompu, ce qui a un effet sur les capacite\u0301s cognitives.\u201d<\/p>\n<p>Pour ve\u0301rifier leurs observations, l\u2019e\u0301quipe du Dr Tsai a cre\u0301e\u0301 des oligodendrocytes humains en culture, porteurs de plusieurs formes du ge\u0300ne APOE. Les cellules avec le variant APOE4 avaient tendance a\u0300 stocker le choleste\u0301rol a\u0300 l\u2019inte\u0301rieur du cytoplasme (la\u0300 ou\u0300 se trouve le re\u0301ticulum endoplasmique) et ne libe\u0301raient que peu de choleste\u0301rol, ce qui les rend moins aptes a\u0300 former des couches enveloppantes de mye\u0301line.<\/p>\n<p>L\u2019espoir de la cyclodextrine<\/p>\n<p>Les chercheurs ont ensuite traite\u0301 les cellules porteuses d\u2019APOE4 avec de la cyclodextrine, un me\u0301dicament qui stimule l\u2019e\u0301limination du choleste\u0301rol. Cela a aide\u0301 a\u0300 restaurer la formation de mye\u0301line. Les chercheurs ont e\u0301galement de\u0301couvert que chez des souris posse\u0301dant deux copies d\u2019APOE4, la cyclodextrine semblait e\u0301liminer le choleste\u0301rol du cerveau, ame\u0301liorer le flux de choleste\u0301rol dans les gaines de mye\u0301line et stimuler les performances cognitives des animaux. Toutefois, la cyclodextrine, tre\u0300s puissante telle qu\u2019elle est formule\u0301e aujourd\u2019hui, pourrait ne pas e\u0302tre ide\u0301ale pour corriger les de\u0301sordres lipidiques dans le cerveau. Mais la mole\u0301cule est porteuse d\u2019espoir. \u201cDes me\u0301dicaments pourraient e\u0302tre cre\u0301e\u0301s sur cette base afin de traiter les alte\u0301rations dues a\u0300 la maladie d\u2019Alzheimer\u201d, veut croire le Dr Tsai.<\/p>\n<p>Cibler le choleste\u0301rol ce\u0301re\u0301bral ne reste toutefois qu\u2019une seule piste parmi d\u2019autres. La maladie d\u2019Alzheimer se caracte\u0301rise par l\u2019agre\u0301gation de deux prote\u0301ines cle\u0301s dans le cerveau, la prote\u0301ine be\u0302ta-amyloi\u0308de et la prote\u0301ine Tau. Plusieurs e\u0301tudes ont montre\u0301 qu\u2019APOE4 e\u0301tait e\u0301galement doublement associe\u0301 a\u0300 ces deux prote\u0301ines. Cette e\u0301tude apporte une pie\u0300ce de plus aux connaissances sur la maladie d\u2019Alzheimer et la fac\u0327on dont elle endommage le cerveau.
